The term is often casually used, fine for conversation, but severely misleading for medical diagnoses, and severely misleading when a person suffering from a clinical depression is told, “pull yourself together; don’t go round with such a long face; you think you’ve got troubles – let me tell you about mine!”</p>
<p>Depression, just like appendicitis, is a clinical diagnosis, and has been subjected to an exact definition. The authorities involved here are psychiatrists, psychologists and others working together in committee to come to agreement on exact terms of diagnosis. They are not involved with defining treatment, only with the definition of terms used in diagnosis. At irregular intervals their conclusions are published in a manual of nearly a thousand pages, called “Diagnostic and Statistical Manual of Mental Disorders.” The current edition is the fourth, and is revised as the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, Text Revision, and in “doctor-speak” called DSM-IV-TR.</p>
<p><strong>The DSM-IV-TR diagnostic criteria for a major depressive disorder (MDD): </strong></p>
<p>A. At least 5 of the following, during the same 2-week period, representing a change from previous functioning; must include either (a) or (b):</p>
<p>(a) Depressed mood;<br />
(b) Diminished interest or pleasure;<br />
(c) Significant weight loss or gain;<br />
(d) Insomnia or hypersomnia;<br />
(e) Psychomotor agitation or retardation;<br />
(f) Fatigue or loss of energy;<br />
(g) Feelings of worthlessness;<br />
(h) Diminished ability to think or concentrate; indecisiveness;<br />
(i) Recurrent thoughts of death, suicidal ideation, suicide attempt, or specific plan for suicide.</p>
<p>B. (omitted)<br />
C. Symptoms cause clinically significant distress or impairment of functioning.<br />
D. Symptoms are not due to the direct physiologic effects of a substance or a general medical condition.<br />
E. Symptoms are not better accounted for by bereavement (abbreviated).</p>
<p>In the primary care setting, where many of these patients first seek treatment, the presenting complaints often can be somatic (physical), such as fatigue, headache, abdominal distress, or change in weight. Patients may complain more of irritability than of sadness or low mood.</p>
<p><strong>Physical appearance</strong></p>
<p>No physical findings are specific to MDD. Diagnosis lies in the history and the mental status examination.</p>
<p><strong>Appearance and Affect</strong></p>
<p>Most patients with MDD present to their physician with a normal appearance. In patients with more severe symptoms, a decline in grooming and hygiene can be observed, as well as a change in weight.</p>
<p><strong>Mood and Thought Process</strong></p>
<p>Patients report a mood state, expressed as sadness, heaviness, numbness, or sometimes irritability and mood swings. They often report a loss of interest or pleasure in their usual activities, difficulty concentrating, or loss of energy and motivation.</p>
<p><strong>Cognition</strong></p>
<p>Patients with MDD often complain of poor memory or concentration. Most commonly, no significant deficits are found on cognitive examination.</p>
<p><strong>Measuring Depression</strong></p>
<p>Defining the term Depression needed a committee to reach unified agreement. How to measure depression, or how to determine degrees of depression has not had the same advantage. There are numerous questionnaires, some self-administered, all depend on the patient’s self-appraisal. Among the commonest reported are the Beck Depression Inventory and the Hamilton Depression Rating Scale. When there are many instruments, it usually indicates that no one of them is completely satisfactory or appropriate to all purposes.</p>
<p><strong>Pain</strong></p>
<p>Depression alone is not a probable cause for pain. In patients with rheumatoid arthritis it was concluded that the depression accounted for no more than one percent of the pain.</p>
<p><strong>Incidence of Depression</strong></p>
<p>In the US, the lifetime incidence of MDD is twenty percent in women and twelve percent in men. Internationally, there are differences in the expression of psychological distress in patients from other countries or cultures. MDD may be expressed as fatigue, imbalance, or neurasthenia in patients of Asian origin.</p>
<p><strong>Underlying causes</strong></p>
<p>We have a lot to learn, but all authorities agree that MDD is a neurochemical disorder. Clinical and preclinical trials suggest in MDD there is a disturbance in Central Nervous System serotonin (ie, 5-HT) activity as an important factor. Other neurotransmitters implicated include norepinephrine and dopamine. The role of CNS serotonin activity in the pathophysiology of MDD is suggested by the efficacy of selective serotonin reuptake inhibitors in the treatment of depression.</p>
<p><strong>Treatment</strong></p>
<p>If your friend, wife or daughter has severe abdominal pain, do you say, “stop whining,” or do you take them to a competent medical person? Depression is no different. Many suicides result from depression, and it must be considered a life threatening condition. It is a condition that requires a skilled person to make the diagnosis, and a team of skilled persons to render treatment.</p>
<p>Depression may be treated with a regimen that includes non-pharmaceutical therapies. Antidepressants may help, but the clinician should also address other symptoms, such as fatigue, and eliminate the possibility of co-morbidities. Modifying diet and practising good sleep hygiene are crucial. Starting a rehabilitation exercise program is important. Behavioral modification techniques and stress management may also be used.</p>
